Eligibility Requirements
Applicant must be:
- Enrolled member of a Federally Recognized Tribe;
- Spouse to enrolled Puyallup Tribal Member
Application Process
Applicant must:
- Fill out the Application for Services
- Provide documentation of enrollment, i.e., copy of Tribal identification card;
