Gather ALL required documents as a PDF (not a picture) prior to starting this application.

You are REQUIRED to submit the following documents:
- Tribal I.D. Card
- Veteran and/or Disability status paperwork, if applicable
- Current Mortgage Statement (For Mortgage Assistance)
- Cover letter of current Signed Lease Agreement (For Rental Assistance) The full lease does not need to be uploaded, just the section that states rental amount, landlord/manager, length of lease, etc.)
- W-9 (if it’s already not on file for Rental Assistance)
Incomplete applications will not be accepted.
