Qualifying factors include any COVID related impacts from March 1, 2020-December 1, 2020
Loss of wages due to COVID:
i.e. laid off, furloughed, reduced hours (children per capita does not qualify) unexpected losses from personal businesses must show what was made from business 1 year ago & can only cover difference
Increased expenses due to COVID:
i.e. grocery stock up due to stay at home order (you do not need to show the difference/increase all grocery receipts are okay), food deliveries (uber eats, post mates, doordash, pizza), educational tools for children, electronics for school/work from home, entertainment for children, work out equipment due to closed gyms, self-care expenses, cleaning supplies, over the counter cold medications, grooming/haircutting due to salons/barbers being closed, personal protection equipment (masks, gloves, sanitizer)
Supporting documentation includes:
Picture of Tribal member holding Tribal IDs & children’s birth certificates
Bank statements (black out any irrelevant information), receipts, proof of loss of wages with number of hours and pay rate
To show increase in utility bills (due to stay home order) must show bills from 1 year ago to show difference in amounts- can only cover difference
Bills that DO NOT qualify are:
Rent/mortgage, loan payments, vehicle repair/upgrades, insurance, utility/cable/internet bills, or any regular year round bills. If unable to pay rent, show proof as to why it was not paid such as loss of wages and denial letter from unemployment. We are not taking self-defense/protection such as firearms, ammunition and weapons
Tips for filling out the application:
Tribal member adult’s name should be on the first line. Do not put the minor Puyallup’s name in the applicant name. List everyone in the household. There will be an option to check if there are multiple Puyallup adults in the same household.
Maximum 2 requests per month. Administratively, staff could not keep up with high volume of small amount requests. This is to prevent “nickel & diming” with daily living receipts.
